Personal Care Assistant/Home Health Aide and CNA
Job Summary
Provides various personal care and related paraprofessional services in accordance with an established plan of care. Provides for the personal needs and comfort of patients in their homes.
Essential Functions
Adheres to Agency policies and procedures
Performs work under the supervision of the of the Supervisor
Performs personal care activities contained in a written assignment by the Supervisor which includes but is not limited to, assisting the patient with activities of daily living, such as:
Personal hygiene (assisting to bathroom or in using bedpan, bathing, care of mouth, skin and hair)
Ambulation
Eating
Dressing
Shaving
Meal Preparation
Performs other assigned activities that are taught by a nurse for a specific patient. These include but are not limited to:
Assisting with the change of ostomy appliances
Reinforcement of dressings (non-sterile)
Assisting with the use of devices for aid to daily living (cane, walker, wheelchair or lift device)
Assisting with prescribed range of motion exercises
Assisting with self-administered simple urine tests for sugar acetone or albumin (Personal Care Assistant/Home Health Aide/CNA may not interpret results)
Measuring and preparing special diets
Measuring intake and output
Taking vital signs (temperature, pulse, respirations and blood pressure)
Application of an external (condom-type) catheter
Assist with application of arm sling, arm splint or leg splint
The Personal Care Assistant/Home Health Aide/CNA will NOT perform any of the following:
Sterile dressing change
Irrigation of body cavities such as enemas, colostomy or wound
Gastric lavage or gavage
Catheterization
Administration of oral medications, eye drops or medicated skin ointments
Administration of parenteral or IV medications
Application of heat by any method
Naso-gastric or gastrostomy tube feedings or care
Tracheotomy tube care
Suctioning of the nose, mouth or tracheotomy
Nail care on a patient with diabetes or compromised circulation
Any personal health service that has not been included on the paraprofessional task sheet
The Personal Care Assistant/Home Health Aide/CNA performs light housekeeping tasks that will facilitate the patient’s self-care at
These tasks include, but are not limited to:
Vacuuming
Dusting
Sweeping or mopping floors
Doing dishes
Laundry
Cleaning bathrooms
The Personal Care Assistant/Home Health Aide/CNA will NOT:
Perform heavy-duty household cleaning such as washing walls, waxing floors, washing windows
Household repairs or maintenance
Yard care or maintenance
The Personal Care Assistant/Home Health Aide/CNA is responsible for documentation of service provided, using the Client Care Record.
The Personal Care Assistant/Home Health Aide/CNA should not contact the physician directly, but rather refer all questions or reports of patient’s condition changes immediately to the Right at Home Office at 810-225-4724.
Additional (non-essential) Desired Functions
Performs other duties as directed by supervisors.
Education, Experience, Knowledge, Skills, Abilities and Availability
High school graduate of E.D. certificate preferred.
Six months experience as a Home Health Aide in health care (in homes or facilities) and successful completion of a State approved Nurse Aide or Home Health Aide
The Personal Care Assistance/Home Health Aide/CNA shall have successfully completed a Competency Evaluation that meets the State licensing law for home health
Must have a sympathetic attitude toward the care of the sick, and ability to deal effectively with the demands of the client
Current CPR Certificate is recommended
Meets the requirements for employees providing direct patient
Read, write, speak and understand English as needed for the
Have valid driver’s license and use of an insured automobile for work or access to adequate transportation
Homemaker/Companion
Job Summary
Provides for the maintenance of safe and clean environment. Performs various housekeeping tasks, meal preparation and companionship activities for clients in their residences.
Essential Functions
Adheres to Right at Home policies and procedures
Performs work under the supervision of the Staffing Coordinator/Operations
Performs homemaking activities contained in the Client Home Record which includes, but is not limited to: vacuuming, dusting, sweeping or mopping floors, doing dishes, changing bed linens, laundry, and cleaning
Prepares meals and snacks for client according to client/family and supervisor’s instructions. May perform grocery shopping, if prearranged with the
Provides companionship for the client including accompanying client on walks, trips to social/recreational activities, assistance with hobbies,
The homemaker is responsible for written documentation of service
The homemaker should not contact the physician directly, but rather refer all questions or reports to the supervisor.
The homemaker will NOT perform any of the following:
Personal care services such as bathing, dressing, and shaving
Any heavy lifting or transferring of
Administering of any medications, ointments, eye drops,
Heavy duty household chores such as washing walls, waxing floors, washing windows,
Household repairs or maintenance
Cut the fingernails or toenails of any client
Additional (non-essential) Functions
Performs other duties as directed by the supervisor.

Working Conditions/Environment
Primarily works in client’s place of residence, travel required. Shift times and days vary with client needs.
